Credit Card Authorization Form
For your protection, if you prefer to pay by credit card, you must complete a one-time credit card authorization form. This will ensure that users of your card are authorized and will prevent unapproved purchases. Please download and fax this completed form to the Optus Accounting Department at 870.974.7773.

Credit/COD Application
If your preferred method of payment is Cash on Delivery (COD) or net terms, please complete this application in its entirety and return by fax to the Optus Accounting Department at 870.974.7773. It is critical that your signature is included on the application. Please note this does not make you liable for any incurred charges. It simply authorizes Optus to verify your credit sources. Repair Tag
To ensure your product is repaired in a timely and efficient manner, please complete a repair tag for each item shipped for repair. Completing this form will help expedite the repair process and hasten your product’s return. The completed form should accompany the product when shipped. If you have questions regarding this form, please contact your Optus Account Executive.

For a faster response on getting your CASH BACK, please print and complete the interactive PDF Bid Specification Sheet, include a detailed list of the equipment you wish to sell, and email back to Optus at purchasing@optusinc.com.
